When you enter the official survey website, you can choose to take the survey online in either English or Spanish. You will need your recent restaurant meal receipt because you will need to enter some information from the receipt to progress through the feedback questions. Before you begin, you will need to enter your personal 12-character code that is found on the receipt, then click go to begin the customer experience and satisfaction feedback survey. If your meal receipt has a 4-digit code you will need to use the alternate feedback website at www.tell-chilis.com if you are within the U.S. or use www.go-chilis.com if you are outside the U.S.
There is only one entry per code. You can view the sweepstakes rules at the website for specific details. You can also view previous sweepstakes winners. See the Chilis Survey website to enter and complete the feedback questionnaire and be entered in the $1,000 sweepstakes.
