If you are eligible to apply for extended benefits, you will need to complete the Application for Extended Unemployment Benefits, you can download the form on the website. the download link are bellow . Otherwise you can obtain this form your nearest local Department of Labor Career Center. On the DOL website, you also have the option to fill out and submit your application electronically.

When you ready to filing claim online then you will need your bank account and routing numbers, If you are a non-citizen, you will need your alien card number and expiration date. Be sure enter your social security number and Personal Identification Number (PIN). If you have misplaced or forgotten your PIN, you can contact your local Career Center to receive a new one, click "Continue"
